Ridership Application

Please fill out this form for every student that will be riding the bus in the 2024-25 school year and return it to your school site administrative assistant or email to [email protected]

 

While riding the bus is free, this application is still required to keep track of students' whereabouts. No student will be let on a bus without the form submitted.
