Request for Facility Use
To request use of a school space, or the district office at 50 Church St. in Quincy, please fill out the application (link below) and submit the form directly to the site where the facility use is requested.
The application will be reviewed by the school principal (or superintendent if the use of the district office is requested) to determine whether any additional custodial or food service costs will be incurred to facilitate the use request.
Insurance Documentation Needed
To all outside users requesting use of any PUSD/PCOE facility – prior to using facility, please provide insurance documentation as follows:
Liability Insurance
User of facility shall secure and maintain, at its own expense, complete insurance including comprehensive general liability, workers’ compensation, automobile liability and errors and omissions coverage, each with policy limits of at least ($1,000,000.00) covering Contractor and its agents, officers, and employees, and with an endorsement naming PCOE/PUSD as an additional insured under each such policy.
Any athletic/sports activity, please increase the liability limits to $2,000,000.
